SHBG is the protein that binds testosterone in the blood. When it drops — common on androgens — more testosterone is free and active.

What SHBG does

Sex hormone-binding globulin binds testosterone and estradiol, keeping them in reserve. Only unbound ('free') hormone is biologically active.

Low SHBG means a larger free fraction; high SHBG means more is bound and less is available, even if total testosterone looks fine.

In enhanced context

  • Oral androgens and high overall androgen load suppress SHBG — a low value is expected, not pathological, in that context.
  • When SHBG is low, free testosterone can be high even with an unremarkable total T, so the two must be read together.
  • Very low SHBG also distorts calculated free-T formulas; a directly measured free T is more reliable.

FAQ

Why is my SHBG low on TRT?

Androgens suppress SHBG, and oral compounds do so strongly. A low value is expected in that context. Because SHBG sets how much testosterone is free, it should be interpreted alongside total and free testosterone.
